Blizzard: Sanctuary will reshape itself between visits

Terrain, enemies, rewards and corruption can all differ on return.

Original HellCodex illustration. Not official Blizzard artwork.

Blizzard has confirmed that Diablo V's world is procedurally altered. When you return to an area, five things can have changed: the terrain, the points of interest, the enemies, the rewards, and the extent of demonic corruption.

The framing matters. Blizzard described this as the world reflecting Diablo's advancing rule, not simply as randomised dungeon generation. We file that intent as a developer statement and the five changing elements as confirmed.

The open question

Nothing has been said about what triggers a change, whether progression is preserved across reshuffles, or how a shifting world interacts with quest design.
